Simsbury Public Schools is accepting applications for a part-time Physical Therapist. Approximately 10-15 hours per week. Hourly Wage is $56.28 - $60.14 commensurate with experience. TITLE: Physical Therapist (PT) Consultant QUALIFICATIONS: Must meet State and/or National Licensing Requirements
JOB GOAL: Assisting children needing PT services in improving their fine and gross motor skills needed for independent or semi-independent educational functioning within the school setting. PERFORMANCE RESPONSIBILITIES: 1. Provides specific services to children as determined through individual assessments, Central PPT recommendations and physician prescription. a. Direct service to children b. Consultation to parents and staff c. Indirect service-monitoring of children under doctors' orders 2. Advises parents when there appears to be a need for a revision of orders or an initiation of a home program. 3. Screens through informal observation to determine if evaluation is necessary. 4. Maintains appropriate student progress records and prepares pertinent reports and IEP's as requested by the Director of Special Services. 5. Attends and/or conducts parent conferences. 6. Serves as a member of the Planning Placement Team when students are being considered for PT services or when such placement or services is being reviewed. 7. Serves as a consultant to other school personnel working with children for whom PT services are provided. 8. Provides, orders or assists in ordering adaptive materials and equipment to be used by student in school environment.
9. Attends, when necessary, agency/hospital clinic sessions with child. 10. Provides agency/hospital, private physicians and Director of Special Services with progress notes at time of child's re-evaluation.
