Job purpose Assess referred patients, ranging in age from 18 years to geriatric who are experiencing rehabilitation diagnosis or conditions requiring rehabilitation. Provide direct patient care to assess patient medical condition, functional capabilities and limitations and restrictions and potential for rehabilitation. Establish and administer a treatment program with specific goals determined according to the patients’ capacity and tolerance under the direction of the Physician. Educates patients and families in appropriate physical therapy methods. Duties and responsibilities · Complete initial and regular patient assessment and reassessment in timely manner documenting outcomes and interpreting of data. · Document outcomes and interpretation of data. Communicate interpretation to Physician and interdisciplinary team. · Plan, coordinate, implement and participate in treatment plan as prescribed by physician and interdisciplinary team. · Administer appropriate physical agent; provide training, education, and fittings with patient and family members; report both oral and written treatment and status regarding patient progress and discharge. · Evaluate care interventions and their effect on outcome; document progress and variances to expected outcomes. · Perform regular checks on patients and equipment. Maintain clean and safe equipment and environment. Maintain quality control of all rehab equipment. · Participate in performance improvement activities in department and hospital wide. · Remain current with literature related to departments for which responsible and maintain an updated methodology, tools, manuals and criteria as necessary and appropriate. Attends all required hospital meetings. · Provide education to patient’s family as well as staff on the specific treatment plan as well as therapy goals for the patient. · Collaborate with the interdisciplinary team to achieve well-rounded care. · Handle job responsibilities in accordance with the Company's Code of Conduct, appropriate professional standard and applicable state/federal laws. · Comply with Federal and State physical therapy regulations · Perform other duties as assigned. Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, EHR, etc.) · Graduate of an accredited school for Physical Therapy. · Bachelor’s Degree, required · A minimum of two (2) years’ experience directly related to duties and responsibilities. Certificates, Licenses, Registrations Physical Therapy licensure, in good standing, within and by the state of work. Must maintain current BLS certification throughout employment. Physical requirements Ability to continuously stand, walk, talk, hear and feel. Ability to frequently sit, push/pull, lift and reach. Ability to occasionally stoop, kneel, crouch, crawl and smell.
