SCHEDULE: 20 hours per week or 40 hours per week (part time or full time available)
Summary of Job Duties : The Physical Therapist will provide on an ongoing basis, physical therapy to Community LIFE participants in an effort to regain or compensate for mobility, strength, range of motion, pain, and/or discomfort with the expected outcome of helping improve the quality of life and independence for recipients of this therapy. The Physical Therapist must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and Community LIFE standards. As the primary contact, the Physical Therapist performs utilization case management for all inpatient admissions and transfers to Skilled Nursing Facilities (SNF). The Physical Therapist directly supervises the Therapy Aide.
Required Education & Experience: Must be a graduate of a physical therapy curriculum approved by the American Physical Therapy Association (APTA) with the minimum of a Bachelor’s degree and at least 2 years’ experience with the geriatric population.
Required Certifications/Licensure : Valid physical therapy license in the state of Pennsylvania.
